Kendriya Vidyalaya AFS Sarsawa Admission Procedure 2020-21

Original and a copy of the following Documents are required to be presented to the school at the time of admission

1. एपलिके शन की प्रलिलिलप / Copy of Registration Application

2. बच्चे की जन्म लिलि का प्रमाण / Proof of Date of Birth of the child

3. बच्चे की दो फोटो / Two Photos of the child

4. लनवास प्रमाण / Proof of residence

5. सक्षम प्रालिकारी द्वारा लदए गए सेवा कममचारी का सेवा प्रमाणपत्र / Service certificate in case of serving employee given by the competent authority (showing number of transfers signed and stamped by the head of the office) (along with address, date of joining, date of appointment and employee code)

6. एससी / एसटी / ओबीसी (एनसीएि) श्रेणी प्रमाण पत्र / Proof of certificate in case of SC/ST/OBC (Non-Creamy Layer) by Competent Authority. In case of OBC (Non-Creamy Layer) Date of issue of the certificate should be from last 6 months

7. ईडब्ल्यूएस / बीपीएि श्रेणी प्रमाणपत्र / Proof of certificate in case of EWS/BPL by Competent Authority /State Government (Date of issue of the certificate should be from last 6 months)

8. आिार काडम / Aadhar Card of the Child and parents

9. रक्त समूह प्रमाणपत्र / Blood Group Certificate by Medical Officer/ Competent Authority

10. लसिंगि गिम चाइल्ड श्रेणी के लिए एलफडेलवट(हिफनामा )/ Affidavit in case of Single Girl Childcountersigned by First Class Judiciary Magistrate of last 6 months

11. लवकिािंगिा प्रमाणपत्र / Proof of certificate in case of disability (DA) by CMO/ Civil Surgeon.

Original and a copy of the following Documents are required to be presented to the school at the time of admission if offered

For Differently abled:

लवकिा गिा प्रमाणपत्र / Proof of certificate in case of disability (DA) as per Govt. Rules (more than 50%) along with the above documents.

Self-Declaration for Distance between School and Residence
I _____________________________ father/ Mother of _________________________ bearing Enrolment ID / Application submission Code _____________________ declare that the radial distance between school (KV AFS Sarsawa) and our residence (as per residence proof submitted along with admission form) is __________ (in words) kilometres. If the above declaration is found to be incorrect, Admission for my ward may be cancelled as per KVS norms.
